Test Form – Epic Ultras

COMMITMENT STATEMENT *I understand the total Epic Ultras Franchise fee is $10,000. By checking this box, I’m committing to remit a 50% deposit ($5,000) within 24 hours of signing the official franchise agreement if I’m selected and awarded a franchise. The remaining 50% balance will be due by January 31st of 2026. Payment may be made via Stripe, PayPal, Venmo, or ACH wire transfer.

TRAINING COMMITMENT *I understand that, if selected, I’m fully committing to attend mandatory Epic Ultras Franchisee Training in Sedona, Arizona, from Thursday, December 11th through Sunday, December 14th. Arrival is expected the afternoon of the 11th, with training beginning at approximately 5–6 p.m. that evening and concluding by noon on the 14th. All lodging and meals (breakfast & dinner) is included in the franchise fee; however, franchisees are fully responsible for their own travel arrangements.

ASSESSMENT ACKNOWLEDGMENT *I understand that the licensed proprietary workplace-based personality assessment I’m submitting as part of this initial franchise interest form is for internal evaluation purposes only. Epic Ultras is under no legal obligation whatsoever to share individual assessment results, scores, or interpretations with applicants or third parties.

LEGAL DISCLOSURE STATEMENT *I understand that this form is not a franchise offer or agreement, and submitting it does not constitute an application for a franchise. Epic Ultras will only make franchise offers through a legally compliant Franchise Disclosure Document (FDD), delivered in accordance with applicable federal and state franchise laws.
